Doctor Referral
We will need a referral from your child's pediatrician or family practitioner to get therapy started.
New Patient Forms
When our Office Staff contacts you to schedule your child's first visit, they will email you New Patient Forms. These forms allow the therapists to review background information and any medical records prior to your child's appointment.
